How hard is it to replace hammer nose bushing
S&W 625 mine is pushing back into to the frame

That doesn't sound right to me. The frame pocket for the bushing is set to a determined depth for the bushing, and the bushing should not go any deeper into the pocket. There are special tools used to remove the old bushing, ream the pocket to remove any burrs (the reamer does not cut the pocket deeper), a special tool used to insert a new bushing in the correct relationship with the hammer nose, and finally a crimping tool to crimp the edge of the pocket around the new bushing.

Sounds like the Bushing has actually broken apart. They are usually pressed in but you will need to see if the pocket is still sized to spec. If it has worn and enlarged you will need an oversize Bushing fit to it.

Just be very careful when removing the pushed in Bushing so you do NOT damage the pocket!
 
If it's a post 1989 625 (quite likely since this model was introduced in 1988) you have a LIFETIME WARRANTY. In that case all I would do is contact S&W and let them handle the repair.
