4" .38 Special Combat Masterpiece. In 1957-58 it would have Model 15 added to the model, but at the time made it was a "Named Model".

No, just 38 Combat Masterpiece.

Welcome to the Forum. Your post will get moved to the correct section soon since this is the Antique Section. That serial number indicate it was manufactured in 1956. Caliber is 38 Special, not 38 S&W. 38 S&W was a short cartridge used in the era of top-break revolvers through pre-WWII K frame revolvers.

SN puts it in 1956. It should have a 4th screw in front of the trigger guard. 5th screw went away in 1956, 4th screw in 1961.
 
Sometimes called the "K-38 Combat Masterpiece," but "K-38" was never a part of its name. There was also a very similar version but with a 6" barrel that was named the "K-38 Masterpiece," later becoming the Model 14. It was marketed mainly as a target revolver. The main difference between the two was the barrel length.
 
BTW, the .38 Combat Masterpiece/Model 15 was a fairly popular police service revolver, and was also the official handgun of the U. S. Air Force prior to the adoption of the M9.
 

That serial number definitely puts it in the 4 screw range, confirmed by your photo showing no upper side plate screw.
