Another variable is the recoil spring. Be sure if you have a cut or aftermarket (non-factory) recoil spring in your gun that the cut end of the spring is to the back, inside the frame cavity.
If the recoil spring has a cut end and this "ubfinished" end is facing forward, the spring can hinder the slide's movement enough to cause reliability issues. Even worse, the tail on the cut end, if facing forward can get caught in the space between the hole in the slide and the guide rod, and tie the gun up.
