model 500 internal lock

S&W customer service not the greatest. Took 4 tryes to get one worth it. I've had the .500 4 " for about a year fired about 400+ 440 gr. rounds out of it w/out problems. Last time fired one round, wouldnt fire again and found that the internal lock had engaged.When i opened the cylinder it came off the revolver.
The 4th cust. serv. rep. said to check the screw in front of the trigger should have a spring attached, no spring. They are happy to send another screw w/spring. My DEADLY curiosity is why the internal lock engages by it self, and the spring breaking. Too bad 500 nut is not around.
