PC shields: do the fiber optic inserts fall out?

I think I have seen maybe just one thread on it here so far (believe it was on a PC); not sure if it was a more unique issue for that owner.

I bought my PC shield about a month ago. The first range visit I fired 250rds. The last 50rd were 125gr +p. I noticed the front sight fell off during those last 50 rds. Unfortunately, I was unable to find the fiber optic insert.
Later, I called S&w and the rep said the received a bad batch of sights from hiviz. The rep said to call hiviz and they would take care of it. I called hiviz and they sent me out two more front fiber optic inserts for free. After installing the insert, I shot about 100 more rds with no problems. I am however switching the trijicon hd xr sights becouse I want nightsights on my carry gun.
I have had a problem with the front roll pen moving out to the right, which prevents me from sliding the take down latch. I even posted a question on the forum but I one really had an answer for me.

It does. Put up a picture of your issue? Is it the roll pin that is lower left below the takedown lever? That is a roll pin and should not move. Maybe they used one too small? Send it back.
BTW how do replace the hiviz insert

Yes it is the pen located located below the take down lever.
The hiviz optic insert actually snaps into place. Hiviz makes a tool to install and remove the insert but I never received one. Hiviz just send me two green inserts, so I used the double lock pen on a handcuff key to push it in. You really don't need the Hiviz tool to replace it, just use anything that's the same diameter as the insert and push it in until he feel it snap into place.
I've heard it happens but not anyone I know of... Have several and have upgraded to night sights on two of them. But all's well with the other one's... but I don't crawl thru stuff or am I what I would call hard on my guns either...
