I've got this holster as part of the package with the M&P9 Pro I just bought. I'm not a fan of appendix carry, so I'm putting it up for sale. This is a Dale Fricke Ehud holster, looks to be barely (if at all) used. It will fit M&P9, M&P40 & M&P357 series full-sized pistols. My guess is it will probably fit compacts as well, but I don't have one to test it with. The full-sized M&P9 is held very securely, but is easy to draw as well.
I believe this may be an older model and the maker has changed the loops to a J-hook configuration.
