We Westerners would call it a "Possibles Bag". It is as stated a Sporran which is a 'man purse'. It is worn more or less in front of the kilt to help prevent an embarrassing exposure from wind bore kilts as the true Scots didn't wear anything as sissified as underwear. ......

My wife is of Scottish descent. She has a picture of a relative in Scotland wearing his full Bagpipe regalia.
 
I have a modern kilt that has pockets. It is made of pretty Cordura and is unaffected by the mind. I'm not near as tough as my few Scot ancestors, I need to wear something under it while working around the campsites!

My Best friend's wife, send to Scotland and had several yards of her family tartan wove, and is making the entire family kilts! I won't be inspecting for proper undergarments.

My second often wears his kilts, they're in USMC tartan. (all our services have a tartan!)
