Yoke comes off my 686-6

This is the second time for this to happen. While pushing the cleaning rod through the cylinder, the yoke came apart and out of the lower portion of the frame. Any ideas on what is going on and how to best get this issue fixed?
After the first time I took it to a local gun smith and had it fixed, but this time should I send it back to Smith & Wesson?

While pushing the cleaning rod through the cylinder, the yoke came apart and out of the lower portion of the frame.
Push hard enough on the cylinder and it will come off. The yoke screw has a spring loaded plunger that will depress allowing the yoke to come out of the frame. Hold the cylinder, not the gun, when cleaning the chambers.
 
I remove the cylinder each time I clean the gun. It takes an extra 20 seconds and saves a lot of wear & tear! It's also a hell of a lot easier to clean! :)

It appears that their is a scrape type bur inside the V area leading to the outside edge. I have cleaned many revolvers before and have never had this happen. When I am holding the revolver, it is in my right hand while griping the cylinder with my fingers. I have also used my drill with a bore brush in it too clean the chambers.

Ok guys I just got it back together, all it took was a flashlight, bent paper clip, and a Gerber tool.

The spring sprung along with that little metal pointy pin so I had to fish it out of the long dark hole.

Got ya. One trick to keep the yoke from pushing out is to replace or supplement the yoke screw spring with (usually) a piece of paperclip to limit the plunger travel. Takes some playing with it to get piece the right length.
