What's the most annoying thing about carrying a Makarov? For me it boiled down to the typical European heel-mounted magazine release which had two problems: 1) it was a ***** to change magazines because a) heel-mounted, b) heavy spring, and c) sum***** was sharp! And 2) being sharp, it would wear out cover garments from the inside and mark up furniture. The solution I found was the FAB Defense PM-G Makarov PM/PPM Magazine Release Grip. It converts the magazine release to a sliding button AR the top of the grip (available in left or right), flush more ergonomic, and has an extended beavertail to prevent "slide bite." The grip is also available in black, olive drab green, and flat dark earth. Mine is the FDE and it rides very comfortably in a Versacarry Commander OWB holster.
A distraction was the unavailability of good defensive ammo. All I could find during the year long "15 days to flatten the curve" was steel-cased ball. That has now resolved itself with PPU brass-cased ball and Hornady Critical Defense nickelplated-cased FTX ammo showing up at the LGS, in this case Sooner Guns.
I have no financial interest in any of the products or businesses I have referenced.

I acquired a surplus Bulgarian Makarov like yours this week. I removed the Cosmoline, and waxed it up good. I am supposed to get a correct grip screw on Monday. I got two pair of grips with it. I like the original reddish Bakelite grips with the star on them better than the black Thumbrest grips. I have studied the Fab Defense grips and like them.

I miss my makarov. I had a Bulgarian one when I was a poor college student, and $180 or so for a reliable 9x18 pistol in a concealable size was music to my ears. I made a kydex holster for it in my parent's garage in the toaster oven with a press I made out of hard foam and plywood. The pistol was a great shooter, and I never felt any problem with carrying it with a round chambered (decocked), which is rare for me for surplus weapons of unknown histories.

As far as the heel release, yeah, I don't care for that one bit, but many of us are also fine carrying less ammo with a slower reload in our J frames, so I didn't worry too much. I am curious about that grip conversion, I've got no experience with a (I assume) rearward traveling button in that spot to drop a magazine, does it feel fast and natural for you yet?

The magazine release on the FAB grip is pushed forward. The black circle on the grip is where the fulcrum point is. Push the release forward, and the bottom pushes the heel release back. It's already natural.
