I realize this is a very old post. The identical thing just happened to me. I took my 11521 apart to inspect and oil. The recoil spring was broken. As I let the guide rod sit with the spring still assembled, the spring began to slowly unwind over the top of the guide rod.

My paperwork came back the same way. I assumed it means the gun was checked and nothing needed to be done.
It would've been nice if they wrote that, instead of a blank form and a signature.

One other thing on assembly. I'm sure your guys were talking about lining everything up for reassembly as the pain in the *** they were referring to. Not much of an issue if a smaller slave pin is used prior to installing the roll pin.
