The failure to extract is exactly what happened. Is there any way to prevent this? Or is it just something that happens on occasion?

So when I take it apart and the spring is hanging lower than when I installed it, that's normal?

I did it 3 times. I put the back end of the spring right in the curved crevasse of the barrel. And almost every time I took it apart to recheck, it was lower than when I put it in.
