The first thing I checked for is leading and/or barrel rifling issues. Nothing noted. I just won't shoot this PPU in the 640, anymore. The Colt KC shot the PPU fine.
The target was a cardboard IDPA target. The target exhibited shrapnel perforations and the rounds were oblong, which meant the rounds were unstable and tumbling.

Just noticed that when loading the cylinder, when rotating the cylinder (while open) the rounds in the cylinder slightly rub against the frame. Is minor contact with the case rim and the frame when loading and rotating normal? My 686+ doesn't do this, but the J-frame does.

I agree, with the mods you are seeking, you might as well buy a 4" or more suitable length and keep that one as is. These aren't exactly falling off the shelves, you know.
When I'm awake, I have either a 60 or a 640 in my right pocket. I use a DeSantis Nemesis. When I'm 'armed' I have an HK P30 AIWB. I always have a J-frame on me, ALWAYS. They are sort of like wearing underwear, it just becomes a part of your wardrobe.
