I am not saying it could happen but the only way I can see it is out of battery as others have said. My only scenario would be after the first shot and the muzzle held hard in as the slide went back the barrel must move up. If it can't it would be out of battery. Long shot at best.

Looking at the amount of wear showing there is no way it's new or is is so far out of spec every time the slide is racked it is rubbing. I have a FS and a shield with 1k rounds through them and the rails don't show that much wear.

My understanding is all picatinny rails are the same size. When you are looking at mm in scope mounts it is for the tube diameter of the scope. If I am wrong it would not be the first time or the last.
