Rifle was clean. Nary a spec of residue on the bolt face. Slides polished, light coat of CLP on everything. Wondering now if it was the magazine. I did clean it up a bit as it had some residue on it. I do not hold the magazine when firing also. Other rounds fired were M22's. No issues.

Interesting. I inspect my mag after I load them to ensure all rounds are staggered correctly. Gun is cleaned each time I shoot. Feed ramp shows no signs of wear or lead/brass buildup. I forgot to add that I believe 1 of the 3 times this happened the rim edge of the round did get "stuck" or...
