There have various solutions spoken about. You can try one of the following
1) Find a torque screw drive, tighten to at least 30lbs
2) Use Blue or Purple Loctite
3) Contact S&W and get a new screw.
I had the issue, I used purple Loctite. No more issue.

My 40c was test fired on 11/7/2012 (date on envelop with spent shell) and it DOES NOT have a nice reset as discribed. You pretty much have no idea when the reset has accored. There is no audible sound or feel.
