I'd have to believe they test fired (and still do) every single centerfire revolver with proof loads to ensure the integrity of each gun. 'Proof loads' are loaded to approximately 150% higher pressure than standard loads for any given caliber and the firing is done inside a proof box to eliminate danger to people in the event of "spontaneous disassembly" in the event of a cylinder failure.
I'd think they'd also fire at least a cylinder full through each gun to be sure the sights were regulated.
Those are personal observations from having spent 21 years in firearms manufacturing, though not at S&W.
They shoot 3 rounds, one out of every other chamber. You can tell if it has only been factory fired by the alternating marks on the front of the cylinder.
