Fuzzy Farrant catalog?

Hello,
I have been doing all sorts of searches for Fuzzy Farants catalog/order form without any luck. I know I have seen it in the order past, but can't find it now.
I would appreciate it if someone could help me.

Look at Mike Priwer's postings- there's a three-fold that shows his order form.

In the early 70s a friend showed me a catalog that I believe was from Fuzzy Farrant. But I seem to remember that the back of the sheet had a diagram of how the maker wanted you to trace out your hand and maybe the revolver butt.
He bought one for his model 25 and it was beautiful. I was just shooting autos and didn't need one.
I wish I had the foresight to get a few. Lol.
I want to try my hand at first altering some grips and then making them from scratch.

I've got a set of Fuzzy's K target style, left side thumb rest/lower right flair, checkered on right side only (for a gun with the cut). If they were not made by Fuzzy they are a dead ringer.
