I have not had much luck carrying a 686+ 4" in an IWB holster. I have a Grizzle SDR made for this revolver. The thickness of the cylinder makes a large bulge at your waistline, which makes the belt sag (Beltman 1-1/2"). Tension of the belt tends to grip the cylinder too tightly, making it hard to draw the weapon. I have VM2 holsters for other pistols, which might work better since they have a steel reinforcing band at the mouth. Alessi and Grizzle have half the wait time as Sparks, which was the deciding factor for me.
On the other hand, I've been very happy with the functionality of a pancake holster. I have an Alessi CQB holster for the 686+ and a 29-8 .44 Mag Mountain gun. They're very comfortable, with just the right amount of retention. My pants don't sag because the belt rides against my body, not the lumpy cylinder. They're harder to conceal, but I plan to use them in the woods, where I can wear a cover garment or carry open.
Tom, at Alessi, recommends against the snap-type CQCS, because that arrangement tends to put too much tension on the cylinder too.
