How much trouble is it, to replace the thin factory hammer, on my 686-4? I'd like to replace it with the wider, target hammer.

Not hard if you are familiar with the internal parts and how to remove them. It would be about a 10 minute job unless stoning of parts is necessary.

I feel very comfortable with the removal, as well as the reassembly. My concern is the fitting of the parts.
What are the chances of ordering a .500 width hammer off of Numrich, and having it just drop in and work like it's supposed to?

I think chances are good it would work fine. Just make sure the hammer is from the correct era.

I've done that same swap on a 686-4, but there was some fitting that needed to be done. The hammer I got from Numrich had a DA sear installed; however, it was too long to function properly. Since I didn't want to try and fit it, I took the DA sear out of my original hammer and installed it in the target hammer. Dropped right in after that and functioned perfectly.

The problem is more likely finding a case hardened L frame target hammer. The correct one would have a silver hammer nose. The only one I've ever seen is on my 686-4 8 3/8s.

You could find one from a 586 but the nose will likely be blued. Also if it's a pre M stamp nose it might not function correctly. I couldn't tell you the difference by looking at them

I found one at Numrich, that's supposed to fit. Don't know how correct it is. But my 686 is just a shooter, so if it works I'll be happy. If all goes well, I'll be looking at swapping the trigger next.
