Short answer take it to a gunsmith. I can trouble shoot some of the things for you but it could be lots of things.
Go ahead and check to see if all chambers "carry up"?
The cylinder stop could be fitted wrong or not fitted at all the the cylinder notches, the spring behind it could be missing, broker or kinked, trashed ext... Do the cylinder notches look clean or has buffing bob gotten his hands on it?
As for the hand, it could be "messed" with already. the spring that sits on the leg of the hand could be installed wrong, broken ext.. Someone could of tried to screw with the hand "window" allowing the hand to set to far forward. If the ratchet is truly screwed up could be from someone fitting a new cylinder incorrectly, or trying to fix carry up incorrectly. take some photos of the hand protruding and the window it comes out of.
Sounds like some one hack-smith'd it at sometime, put it back together wrong, it could be from wear to a point but doesnt sound like it totally
