For M&P 15 owners: how is your castle nut staked, and has it ever come loose?

I handled a Sport today at a local shop, and noticed that it had a very poor staking job on the castle nut. It looked like it had been done with a flat blade screwdriver. One stake provided no benefit at all, and the other had only deformed a tiny bit of metal into the notch on the nut. I'd like to know if this is a common occurrence, or a one-off.

It's tough to say without seeing but it doesn't take much to make a stake. I've seen builders do it with spring loaded punches (usually leave a light mark), hammer and punch and even a screwdriver. Most builders do not stake the castle nut in more than one slot.
