A hanky wadded up in the bottom of the pocket can do a great job breaking up the shape of a pocket holster and making it look even more nondescript.
I'm sure that some of us do get "made" occasionally, or at least raise someone's suspicion. It's just that the kind of people that do notice such things generally aren't the kind that will be alarmed because they carry also. I was in a local hospital rehab facility the other day and made a guy carrying a very large auto IWB on the left side. I'm thinking he had a mouse gun in the front pocket of his hoody. He didn't have a clue that I had his number. Everyone else in the room was oblivious.
