Custom sheath maker recommendations?

Recently picked up a 4.5" Boker Applegate-Fairbairn and the factory sheath is, to put it kindly, ridiculously large. It's twice the width of the blade.

So, any recommendations for a custom maker who can make something slim and unobtrusive? I'd also need the belt loop to toggle with respect to the sheath so I can pocket carry.
