Cylinder getting stuck

I've got a 686 + TALO with a couple hundred rounds through it. Every now and then I can't get the cylinder to swing out from the frame. I don't see anything that looks out of place or damaged at all. Occasionally when this happens, I eventually get it out but won't latch when I try to close it. After some fiddling with it and the release latch, it goes back to normal.

Anyone experienced this before?

For the record, I ground down the numb on the ILS.

ETA the ejector has not backed out.

Looks like you already ruled out the ejector rod backing out? Second option would be fouling under the ejector star - A little bit goes a long way.
 
Mine did the same. and I think it was a combination of 2 things... My ejector rod started to come loose. I'd tighten it and it would shoot loose after a short time. That woulda "kinda, sorta" jamb the cylinder closed but it would open with some fiddling. The second, bigger issue was, when closing the cylinder, if the gun was pointed down the cylinder would scrape the forcing cone. After shooting it for awhile like this it finally went back to S&W and came back tight both ways.
 
Im hoping it doesn't have to go back due to a bad experience with their customer service. Even when the cylinder comes out normally, I can feel it hanging up on something.
