Fullsize 9mm w/ FSS Trigger kit & FRE / exactly 250rds fired usuing old magazines / lubed with fireclean. On occasion (5%), the slide will not return to battery after firing a shot. This is Not acceptable….any thoughts?

This is typically an indication that the recoil spring is weak. If your spring is factory, and only 250 rounds from new, it may suggest a defective spring.
 
I've replaced factory recoil springs and it still does this.

Happens randomly, sometimes after 1st shot. Also happens every time I load in chamber by hand and hit the slide stop and/or slingshot method.

I don't want to send back to s&w b/c I've read that no matter what you tell them or if you write in full details they will tell you that they never heard of this before AND it takes 2-3 trips before they actually fix it and they very vague in the details of how they fix.
 
when I owned that pistol I had the same problem. I had to experiment with different factory loads. I sent it back - why waste the time when it doesnt function half the time? if its a carry your in trouble
 
Can you post a pic of what's happening? There are different levels of "not returning to battery" and we need to know exactly before we can truly help.

For example, is it doing this:

...or is it something else?
 
If you only have 250 rounds through it, I'd shoot it some more before sending it back or fiddling with the recoil springs.
 
I have a full size M&P 9 back at S&W for failure to feed issues. If this is what yours is doing, I'd send it in to be fixed.

Is the case mouth buggered up on the failed cases?
 
I have a full size M&P 9 back at S&W for failure to feed issues. If this is what yours is doing, I'd send it in to be fixed.

OK, well, what you have is not what we pictured above. This is a Failure To Extract. There are several things that could cause this.

Did you watch the follow up video? In that he says that hi recoil spring was broken. That will definitely affect how the gun operates.

Also, look at your extractor. If it's chipped or bent it could cause weak ejection. Then look at the ejector. It could be broken.

The snap cap you pictured above should be thrown away. Did you try to put it in the chamber first and then close the slide?

It's on its first and hopefully only trip to S&W. No update from them yet. I'll report back when I hear something or after trip to the range with the fixed gun.

And yeah, that snap cap looks like it was already in the chamber when the slide was slammed shut on it. Very bad for the extractor.

After watching the video you posted, we don't have the same problem. Mine is failure to feed not failure to eject.

+1 not on loading a cartridge by hand. Its never a good idea. The best way is to load it into a mag and insert the mag. Then rack the slide and let it load the normal way. Putting a cartridge in by hand tends to bugger up the extractor. Its very possible that the slide might not return to battery if the extractor is messed up.

Take a look at it.
 
Thank you ALL for your replies. I forgot to add an important detail.....I have a brand new KKM barrel installed. I just got done polishing the feed ramp and chamber...and cut 1.5 coils off extractor spring. I'll purchase another one just in case. Plan on hitting range in my lunch brake. ...will provide update shortly.
 
100 flawless rounds.....I now think that my issue is simply due to the combination of brand new FRE, Extractor spring and barrel. ..mostly due to Extractor spring needing to break in.

Hopefully no more issues. Thanks again to all who replied.
