heirloom warranty certificate for elite series

Please help! Does anyone have an heirloom warranty certificate (the one you fill out and send in) for an Elite series shotgun. Any chance you could scan it and e-mail a .jpg or .pdf of it?. We need it for a customer. I can mail you some S&W patches in return. PM me if you can help. Thanks very much.
