Identify 38 spl.

S145945 would not be a SN for a S&W .38 Special K-frame M&P revolver. It may be an N-frame .38/44, in which case:

S140,000 – S149,999……Late 1955 – Early 1956 (manufacture)

Does it look anything like this? (could also have an adjustable rear sight)

Yes, except barrel has a rib, front sight is on short ramp, blade squared off at rear. (Patridge?)
 
Hand Ejector, SN# S145945, blue, 6.5", adjustable sights, 5 screw.

Based on your description you have a 38/44 Outdoorsman Model of 1950 from circa 1955-56 . . . a very fine large-frame target revolver. We would enjoy pictures if you're able to post them.

edit to add: here is a picture of S141007 equipped with target hammer and trigger.



Although also serial numbered in the '55-'56 series, it didn't ship from the factory until November 1958 . . . not unusual for Outdoorsmen in that period when demand had fallen off.
