Reloads or factory? Round nosed, flat point, hollow point? Clean gun or dirty? One mag or several?

It happens. There are several reasons why. More information is needed.

Factory ammo, 40. Fiocchi Fmj, Brand new M&P so its clean, everything original, out of box

Does this mean that the shipping preservative grease was removed and the gun lubed in accordance to the owner's manual? The preservative is tacky enough by itself. Add carbon and powder fouling and it gets like glue and will definitely slow down the slide.

Hard to be sure from that angle but it looks like the nose bit into the bottom of the feed ramp - could be a mag lips issue and the round not hitting the ramp at a high enough angle.

Yes I have clean off the shipping preserve and followed the manual, going to test again at the range...I will update thanks!

Yea, I've looked around for similar problems, it may be an extractor issue...posting pics

Here are a couple more angles of the failure, looks like the extractor is no doing its job, at least for this ammo (Fiocchi), Winchester, Federal, Independence cycle fine when cycling the firearm, but failure happens on Fiocchi rounds, I was hoping this firearm could use any kind of ammo, but as the manual states...try other ammo...going to try it at the range again, (without Fiocchi) and see if failures continue....update to come.

Looks to me like something is out of spec. I'd guess the Fiocchi ammunition's case head or extractor groove. Since it feeds other brands of ammo, I don't think I'd waste time sending it in. You could try a different lot of Fiocchi.

OK, as a noobie, I do appreciate everyones input. I used some Break Free solvent on the extractor and the feed ramp, and went to the range today. 300 rounds PCM, WInchester and Independance - 0 failures, I think this M&P is staying home. Thanks guys! Stay safe!
