New 686P divot in cylinder?

Picked this 686P up today and found this divot on the cylinder. It looks like a machinist tapped it with a drill bit and said "whoops."

I have 6 other S&W revolvers and none of them have this. Check out the picture and you'll see it looks like they tapped it twice with a punch at the edge? If this is a flaw it's going back, but this is the first 7 shot I bought. The punch taps leave me thinking this might have been done on purpose?? Any thoughts will be appreciated. Thanks.

It'll be interesting to see what others say.

It almost looks like they were trying to stake something into that drilled hole.
 
I have a 686 Plus with 4 " barrel and there is nothing on the cylinder like what you have pictured.
