Old Revolver With No Model Number

With that checkering? Nope. It's been welded up to enlarge it and poorly checkered. Functional but not pretty.

So this is interesting and perfectly plausible, as my grandpa was a bowyer and a fletcher and a woodworker, and I even made some leather holsters with him when I was little…so the thought of him tinkering with the hammer is a very realistic scenario.

If that hammer spur was modified by your grandfather I would consider that a bonus to the heirloom factor.

Am I understanding correctly that “.38 S&W SPL” is not the same as a standard .38 Special round? Or will this pistol fire regular .38 Special?

All .38 Specials are the same, Colt and S&W.
.38 Short and .38 Long Colt are the same dia. as Specials but different lengths and will work in .38 Specials.
.38 S&W (not Special) and .38 Colt New Police are the same dia. but larger dia. than .38 Special and will not work in .38 Special guns.
Have I got you confused? :D Larry
