It took me a few minutes to tweak the ejector tab, and Voila!, no more issues. I have no suggestions on the trigger issue, mine didn't do that. Maybe sometimes you're not releasing the trigger past the reset point? Is the new round chambered and action/slide fully closed? Dirty chamber? Happen with all ammo or just certain kinds?
