If he was showing dementia in his final days then collapsed suddenly, my money would be on a stroke, which goes with the whole gout/hypertension suite of conditions.
I'm somewhat fortunate in that i first experienced gout in my early 20s no doubt due to my Southern fried rearing, plus my Dr. said I'm a "under-secreter" of uric acid). I got on the health wagon and had my last severe attack at age 30. They say it takes about a decade for uric acid to build to the point of causing an attack, which makes sense cause it took me almost a decade to get past them.
Yes it is unbelievably painful. I had a couple of nights where I couldn't stand the weight of a sheet on my foot. Even then it throbs constantly.
Now, at 55, when I have joint problems or other pain, I make sure not to tell the doctor I have a history of gout, otherwise they'll just say "oh, its probably gout then"
Or I guess you could say Sam let his chambers get fouled to the point that the cylinder seized up.
