Slide stuck partly retracted on M&P40

Does anyone know what's wrong/can help me fix my M&P40? See atch photo. The slide will retract fully, but will not move forward any more even with the sear deactivation lever & take down lever down.

Can you put the sear deactivation lever & take down lever in the shooting position and rack the slide?

Is the Slide Stop Spring in its proper place?

Fix Your Shield Before It Fails!!!

insure the chamber is empty, and pull the trigger. My Shield 40 does the same thing. The sear deactivation lever doesn't quite release the sear all the way. Again, INSURE THE CHAMBER IS EMPTY.
