I have a Model 4506 with adjustable rear sight.
Is there a replacement sight blade with a wider
slot opening? (failing eyes).

If it's the "winged" factory sight they actually use the same sight blade as the revolver. I don't know of any wider slots available but there might be. You could open it up yourself or it might be easier to check with Novaks and see if they make a narrow front, you would need to measure the height of your front sight for them(usually .232"-.245").

If they take the same blade as the wheel guns do, then yes, they are available in different notch widths and some variations in height as well.
