My rear sight base is bent.
I believe I might have accidentally assembled it incorrectly and the elevation screw bent the blade.
I torqued it raising the blade off and up.

Does anybody know what assembly I should replace this with?

The blade is the piece with the square notch. The tang is the part that goes in slot on top strap and is held in place by a small screw above the front of cylinder. If you completely removed the sight assy and the rear portion of your sight is now way high, make sure you have the small nut on bottom rear of assy in the slots for it.

Like stated It would not be easy to bend the tang as the screw elevation and attachment screw are small and the tang is actually a spring with a shallow bend in it. You could raise the sight completely out of adjustment and not hurt it as long as elevation screw starts back in its nut

That’s exactly what I did a few years ago.

Just evidence I have not fired that gun all that much.

Hi. I had the same problem with my old S&W 686. I ended up bending the rear sight by over-tightening it. What worked for me was ordering a replacement rear sight assembly directly from Smith & Wesson. They have good customer service and helped me find the exact part I needed. You can also check Brownells or MidwayUSA for replacement parts.
 
If you remove the screw in the tang then run the elevation screw up to the point it only has a couple threads left in the nut it is easier to install the nut. It can be trick as the nut can and will turn if not perfectly lined up with the slot when you try to slide it in. But once the nut is in the slot the tang might still be turned up a bit at the front. One of those small plastic clamps is useful to hold the tang down while you install that tiny front screw. I use a small piece of brass and tiny vise grips.

Here I am drilling and tapping a frame for that screw

But anyway, I just bought a new style rear sight assembly from a forum member for my parts pile. IF you find yours is actually messed up, PM me I will tell you what to check to see if yours matches it and if so will trade you it for yours plus postage and $25

If nothing else , try Fusion Firearms . They have quite the collection of front and rear sights for S&W revolvers and semi-autos . They've gotten quite a bit of my business , great people .
