Okay ... here's what the issue was, got it back from SW. They replaced the slide stop as it had taken a beating when the barrel nut came loose. I had a rubber o-ring on it, but that was not enough to keep it tight.

Soo ... I'll add the wrench to my range bag and check it before & during & after use. I usually shoot 200 rounds at a time, have 1,200 rounds through mine thus far.

First thing I did was get five mags. Then I check the barrel after shooting 50 rounds and tighten IF necessary; I have to reload the mags anyway. O ring solved the barrel coming loose issue.

2,000 rounds and loving it.
 
Another trick that you can do is torque the bottom barrel screw to 40 inch lbs when you apply the No. 61 O-ring on the barrel bolt. That will keep it tight until you want it off. Also keep a eye on the ejector as sometimes they can get slightly bent which you can straighten back out yourself with a small pair of needlenose pliers. CCI SV ammo is one of the more accurate bullets to shoot thru that pistol as well.
