I called S&W and the hammer nose and bushing have not been replaced on this gun. Got it all packed up and FedEx is coming to pick it up today. Thanks for letting me know about that @murphydog! :)

Thanks for the info Kernel! I would agree it was more of a defect on the part of the primer manufacturer. I believe I did see the piece of metal behind the trigger. Now I'm going to have to pull it out of the safe so I can verify :)
