I need a recommendation for an OWB belt pancake leather holster for a 3" 686 plus. I know about the Simply Rugged Sourdough Pancake but would like some other options to consider.

OST as I will be in the same dilemma on Saturday when my 5" 686 plus arrives from PSA.

Are you perhaps asking for brand recommendations? Given that you've already chosen the pancake style, so someone who (1) makes pancaked holsters and (2) has a fitment for the 3" 686 and (3) someone has personal experience with it and (4) likes it as opposed to hates it. Unclear from your query.

Oh, ignoring what I think you're asking for, and instead suggesting what design details will carry the revolver well: look for the cylinder of the revolver to be placed coplanar with the trousers belt (I assume). Higher will get wobbly, lower will be harder to hide. The last Galcos I saw were WAY too high for such a short barrel:

I like pancake holsters fine, but prefer the Askins Avenger style. It takes up less space on the belt and doesn't close up when you draw the gun, which makes reholstering easier. My favorite in this category is the PMK by Milt Sparks. They can be tough to find though. There are some vendors listed on the Milt Sparks website that might have some in stock. Otherwise, you can order one and wait a while to get it. The PMK design fits what Red prefers, as he stated above. BTW, Red is worth listening to when discussing leather.
 
I have several Sourdough holsters in various sizes and barrel lengths, but I picked up a lined Wright Leather Works Predator pancake holster off ebay for my M58 and I find it more comfortable and easier to draw from than the Sourdoughs. Not that I'm getting rid of any of them.
 
Check out 1791 Gunleather in Miami, FL. I just bought this OWB holster for my 3" 686-6 Plus (L-frame, RVH4.3, classic brown)...received it in one week, $62. It's a Florida company that uses quality USA hides, and the holsters are actually made in Mexico by a high quality boot and leather company. I'm very happy with this one and you can't beat the price.

MTR Custom Leather has several options.

I like the Belt Slide Master Carry. The cylinder rides near the belt line for good balance and draw on this pancake style holster. A thumb break is extra on this model. It conceals well for an OWB holster.

Which reminds me, because this one is an asymmetrical pancake -- that is, unlike Roy's original, the outermost pocket is cut larger than the backside, and all the moulding is to the outside. I do believe that it was Galco who did this first while still known as Jackass (Gallagher told me he named the company after himself).

Anyhoo, what the asymmetrical configuration does is eliminate the binding of the old style on the draw, then the interference with the holstering (it's not 'reholstering', we don't 'refire') because the belt has drawn the holster shut. Not everyone follows this improvement that concided with the original, with both being 1970s.

So: when choosing a pancake, I suggest you all will be happier with the more shapely version (for my versions for Aker as a design customer, I named them "flatsiders" because it really points out the distinction from the originals):
