Anyone Change How They CCW w/Age?

I'm 78, with degenerative disk disease, sciatica, and arthritic hips and knees. I'm right-handed but my right leg is almost 3/4 of an inch shorter than my left. I walk with a cane and nowadays carry an oxygen cylinder in a backpack and I have the overhanging gut and Old Guy's Disappearing Derriere Disorder.

My experience mirrors yours, except that I was never a cop.
I carry a J frame airweight in a front pocket holster. I can also carry in a shoulder holster or a fanny pack. But IWB or OWB carry of a handgun has me tightening my belt to keep my pants up. I recently suffered a 6 week bout of sciatica because of this.
My problem is that my belly forces the belt downwards, and my fanny is not big enough to keep the belt up. So my pants tend to slide downwards unless I make the belt really tight. That's where my problem starts.

Hey I'm not alone! My belly gets in the way too.

I never even thought about it until I saw this thread but yeah, I have changed it up. I used to hip carry a G19 in a Serpa holster and even tried a shoulder rig a few times. In summer time, I often used a "fanny pack" style concealment holster for it, or just carried a smaller pocket pistol.
I'd tried several IWB's that I didn't like or just ended up not being comfortable but, they've improved greatly over the years.
Today, I carry a Shield in a very slim Kydex IWB, at the 4 o'clock position. Carrying my smartphone at the 3 o'clock position, also helps conceal it. It's so comfortable that I can "almost" forget it's there.;)
The only downside to carrying in this position is, it would be difficult to get to while sitting in my car, if I needed it. For this reason, I rigged up a holder between my seat and the center console that my holster will snap into so now, my pistol is readily available if the need should arise.
Here's the holster. On the backside, it goes almost all the way up and even covers the safety.

Looks a lot like the new kydex holster I got from Amazon for my Shield. Good holster but yeah, for the car you need faster access.
 
Switched from 1911s to J-frames because of arthritis in my left thumb. Made it difficult to rack the slide. I carry more often though because I'm not as good at defending myself as I once was. Carry position depends on weather and where I'm going.
 
I had back problems and my Witch Doctor said to wear suspenders, I did, and pain went away in a few months with careful exercise...pocket carry of an auto then but have changed to a 642-2 in a Galco hornet holster reversed behind right hip for butt first carry (they don't make a left handed holster which would be better) it weights 16.7 oz loaded and I can carry all day without noticing it. Changed to a revolver because of left hand problems jacking a slide... and an inside the pants holster is uncomfrontable for me with any handgun...
