Bolt not locking back on last round

Has anybody had a problem of the bolt not locking back on the last round? I have three ten round mags with the blue followers and one twenty five with the orange follower.It doesn't matter which I use, the bolt still goes foward and I have dry fired it more than once. It also has fired a two round burst. Serial #DTUxxxx bought in Januray. I am now waiting for the UPS label from Smith Ron

it does need to go back for the burst fires but the bolt hold function is prob just some grit under the bolt release button... what ammo are you firing.. cheap ammo can cause all those problems
 
A sticky bolt catch can prevent the magazine follower from raising it when the magazine is empty. Crud or just tight from being new.

I am using CCI Velocitor, hypervelocity 40 grain copper-plated. It started to happen at about 50 rounds.
 
May be a stupid question but did you pre clean the gun before you shot it.. the heavy grease can cause problems... if you did send it back and let s&w handle it
 
I hav had this happen once ... wasn't too concerned about it actually... I shrugged my shoulders, said, "huh, the bolt didn't lock open" clicked in another magazine and got back to blowing $4!+ up ...
