Picked up a S&W auto w/ box at a show this weekend and it has a letter (Dec, 1990) in it from the S&W T&E Program Manager to Law Enforcement Evaluator mentioning, "the enclosed Department Handgun Evaluation Form."

Unfortunately the form is not in the box. Does anyone here have a picture (or scan) of one? I'd love to see that form.

I've never seen a letter like that before. I am sure someone out there has copy of the form. Good luck on finding it.
 
When my agency T&E'd for our new duty pistol, we sent requests on letterhead to each manufacturer. They all sent letters with very similar "standard language" in their letters, and S&W sent that one to us.
We used a point system to score each pistol on a scale of 1-10 in ergonomics, accuracy, reliability, durability, ease of maintenence, weight, capacity, and so on.
The M&P .40 won in every single category so they have been our duty pistols since about March.
Every officer loves their S&W M&P.

Did S&W send you a form or did you just rate it on your own.

Not surprised the M&P40 won every category. I love mine and I'm a die hard traditional 3rd gen auto guy.
