Fairly new handgun shooter using an M&P 9. Every 6 rounds or so I end up with a ftf. Not sure what is causing this. It wasn't an issue until recently. I did install the apex dcaek recently. I didn't notice a problem until after the install. Could I have messed up something? I know the most likely culprate is limp wresting, but I don't think that I am. I am using a thumbs forward grip. Is there a way to figure out if it's me or my m&p?

For some that will keep all the various acronyms straight is the FTF a failure to fire ??

If its a firing issues is the primer dimpled but did not fire . Clean the striker area and parts and leave dry , change ammo to a higher quality product , speer lawman for example .
Failure to feed - buy a case gauge to check radom rounds in and learn to do a plunk test to be sure any new brand or lot number is in spec for you barrel . Try a better ammo even the Premium HP you plan to use and change mags and number them so problems can be tracked if mag related .

Limp wristing can cause a round not to cycle and fully chamber but not likely . Maybe a well seasoned shooter can give your pistol a try and using ammo of there choice .

Factory, handloads, bullet style????? Can make all the difference in the world. Magazine has been dropped and damaged? Disassembled magazine and replaced spring backward or not properly seated in the follower? Maybe never have cleaned the magazine? Gun not adequately lubricated?

Your question is akin to asking an auto mechanic "My car won't start, what's wrong?" without providing at least a modicum of pertinent information!

I used to get this question on the phone all the time during my working years, after I told them I couldn't answer until I figured out what was wrong the next question was "Then, how much will it cost to fix it". I'm sure glad I'm retired and don't have to deal with this stuff anymore.
