I have to agree with the people who are thinking the bolt isn't sliding into battery properly. On that first round with a loaded magazine properly seated, pull the charging handle all the way rearward and let go of it from there, allowing it to move forward on it's own.

This can be practiced without the presence of live ammo as well. Bolt should be well lubed, and a light coat of lube/oil on the entire BCG is advised. Good luck and happy shooting!
 
You have a 15 T so you have a forward assist. If round doesn't fire smack the forward assist that's what it for, to seat a round that isn't fully seated. Could be grit in there or just not fully chambering like stated. The BCG does look used and not new from your picture with all the streaks on it but that could just be the low picture quality or hand charging..

If it doesn't fire then how about a picture of the lower with the hammer in the fired position. You got all the other answers that can help, If these don't work send her home.
 
I do not advise smacking the forward assist. Using anything more than thumb pressure means that you're not assisting the bolt to seat, you're forcing it. Unless you're in a life or death fire fight, there's no overriding reason to force a round to chamber.

The OP can have a dirty bolt, burr in the firing pin channel, problem in the fire control group, out of spec chamber... It could be anything, and were not there in person to inspect the rifle's operation. IMO for the problem he's having he would be safer seeking assistance in the real world and not here in The Matrix.

I am going to have to apologize the problem was me,
I used to have a Hi Point 9mm carbine I was used to letting the charge handle go back with my fingers holding it , and it would chamber the first round and chamber and fire like it is supposed to. I charged this one the same way.
Getting to the point I just came back from the range I let the charging handle go back on it's own and guess what it shot
and I mean shot this gun is beyond my expectation I only brought two magazines with me to test, one loaded with 5.56 the other .223 but I just have to show you the results on the 50 yd range stock sights using a bag
I knew S&W wouldn't disappoint me.
