Issues With my S&W M&P 22 Compact 3.6 Inch Barrel

I recently acquired this handgun and like it but there are a couple issues. First, the rear sight plastic blade kept falling out until I put blue locktight on the screw that holds it in. Seems to work fine now. I think it is a bad design. Second, in my last range session the first round would not fire consistently after inserting the magazine and racking the slide. After ejecting the first round, it fired fine with the remaining rounds in the magazine. This happened with both full and not full magazines. The ejected round does not seem to have a firing pin strike on the case. The gun was cleaned thoroughly before this session. Any thoughts before I give S&W customer service a call.

The rear sight is a cheap crapola design.....First time shooting mine the windage screw almost fell out......I called S&W.....Tech on the phone had NO ANSWER for a fix.....I rezeroed it and filled the hole with RTV silicone sealer.......So far it hasn't moved again......Mine fires everything I run through it........Including using a Dead AIr suppressor on it...........Maybe some day someone will make a decent after market sight for it.
