I have a new M&P 2.0 compact and I had an issue where after the slide was held open I would insert a mag, rack it, and it would not fire the first round. This happened a couple of times over a few hundred rounds. However, after this happened a couple of times I would rack the slide harder and the problem went away. This will never happen if the slide is closed and I insert a mag and rack it. I have a Glock 19 and I have never had this problem. Does the M&P require a little more force to correctly cock the striker when it is open and insert a loaded mag?

I don't think it is out of battery since the trigger did click. I suspect that the striker was not cocked enough and it caused this.

Was there a mark on the primer? What ammo are you using?

As for the striker, it is "cocked" when the slide goes forward and the striker tab engages the sear. It is backwards to the way hammer guns work.

There was no mark on the primer.
 
How does one "rack the slide harder"? Are you riding the slide forward? Does the slide go fully into battery?

When the slide was locked back and I inserted a mag I am now pulling harder. I noticed when I wasn't pulling as hard it had the problems of not firing the first round. I don't know if it was out of battery, but sounds like that might be the case. I think I might have been riding the slide a little bit which caused the error.
