I wonder if you're getting some loose, unburned powder particles underneath the extractor star? What have you been doing to get the action to free up so you can start shooting again?

Perhaps the trigger is not returning fully/completely. Could be debris or old lube, etc, or someone has modified (cut) the rebound spring, or installed an lighter aftermarket spring.
Another possibility is the bevel on the stop, or the hook on the trigger is worn or improperly fit causing a malfunction of the cyl stop. (late)

There are several other issues that might cause what you describe. Hard to diagnose without actually function checking the gun.

If you are able, a good cleaning and relubrication is in order, checking the condition of internal springs and parts.

Thanks rj m6120 good point, the boys at the club empty chamber spin it and fiddle around clean check they aren't sure ,the trigger gets alittle gritty some times too but when it works it gets really nice.

I second the idea that the internal lockwork needs a good flushing to get rid of gunk, then a very light relubrication. This is based on the "the trigger gets a little gritty sometimes" comment.

Thanks everybody for your help. Afriend of mind stripped it down and put in in his ultra sonic cleaner [wow] the old oil etc was unreal, he said it may had never been cleaned as good like that before works very well now thanks very much.

When acquiring a new to me revolver ( I don't buy new production Smiths ) I make it a habit to detail strip it down to the last part, clean, inspect, lube and de-burr it prior to shooting it. This way I KNOW the Revolver has been gone through and is good to go. Any issues are addressed and corrected prior to shooting it and I know exactly what lube is in there and what condition the internal parts are in.
