Model 24-6 cylinder lock-up

I have a 24-6 in nickel that's just a few months old. Recently cleaned it after several range outings, including removing the cylinder. When I was getting ready to shoot the next time I found that the cylinder was locked up tight in the frame. It would turn and index properly, but I could NOT open it. The release button was all the way forward.
I had to have a gunsmith get it freed, and he could not pinpoint a cause.

Does this happen often?

I beleieve the clue is in your statement about removing the cylinder for cleaning. You may have expereinced the cylinder unscrewing fromm the rod. When that happens place a piece of paper between the cylinder stop and cylinder and gently rotate the cylinder and tighten enough to open the yoke. Once you get the yoke open you will have to properly tighten the cylinder to the rod.
