Revolver alignment pin problem/???

A question for you S&W smiths out there. I seem to have lost one of the alignment pins from the cylinder of my Mdl 10-10. I think it must have fallen out. That or broken off down inside the cylinder. I can stick a paper clip down into the tiny hole in the cylinder about 1/16th inch. I have never had this happen before, is it something rare, or fairly common? I discovered the problem while cleaning after a range session today.

My question is: How tight are these two pins in the cylinder? Is it possible for one to loosen and fall out? Replacing the pin doesn't seem like it would be a difficult job, but how to I make sure it stays there?

Any suggestions and/or comments are appreciated.

i had a 1917 s&w that was missing the pin, i found a pin that was the correct size and cut it down after measring a pin on another gun. i crimped the end of the pin that goes into the cylinder with needle nose pliers slightly and tapped the pin in place, and then took a sharp punch and staked it in place. never had any more problems with it.
 
THis is not an uncommon occurrance. Call S&W, tell them what happened; they might just send you a couple extra for nothing. I know they've done it before.

Set new ones in with a brass drift; might want to use some blue Loctite on it before you install it so it won't come out again.
