I sent a PM to Engineer1911 to ask where he located the form. In her July 1, 2017 post Sheryl stated that the cost of the badges was $55.00. Hope that there has not been a price increase.

I just received a note from Sheryl. I am also adding the information from Doc44.

I don't believe that there is a form available for the SWHF badge. However, this procedure should work:

In whatever form, handwritten or typed, send a note to Sheryl to request your SWHF badge at the appropriate level:

a) Benefactor (Gold Tone)
b) Patron (Silver Tone)
c) Member (Copper Tone)

Specify the name that you want on your badge, and as Sheryl reminded me, be sure to include a check, payable to the SWHF, in the amount of $55.00. The address is:

Sheryl, I am really excited, my badge arrived today, and it is georgeous!!! Heavy, extremely well made and finished, and absolutely stunning!! Certainly a fitting tribute to our great Historical Foundation, which is engaged in such critical preservation work!!

I only wish that my poor iPhone photo could do justice to it!!...
