A S&W being dropped on a Naval ship from elevation onto a steel deck causing it to break the hammer stud and fire is supposed to be the reason for the second system hammer block being added.
Yes. The incident was reported to S&W in late summer, 1944. The engineers were put to work on the problem and by early December, the new design had been perfected and approved by management. Production was implemented in December and the first revolvers with the sliding hammer block safety shipped to the Navy (via the Army) in January, 1945. An S was added in front of the V on the serial number to denote revolvers that had the improvement. The sideplate was also marked with a small S on the outside near the rear, because the sideplate had to undergo additional machining on the inside to accommodate the new design.
